Interagency Meeting Minutes 11/13/14

*Interesting facts- In Warren, Washington, Clinton, and Essex Counties smokers are on the rise in rural communities. Individuals suffering from some sort of mental illness account for 1 out of every 3 cigarettes that are smoked.

Code Blue- Open Door Website has a list of fundraisers that are going on to support the mission of the Code Blue Emergency Shelter. The Kick off starts November 15th with Stuff a Truck. Please visit Open Doors website for more information.

Genny Obrien spoke about SNAP/HEAP benefits registrations being completed on-line. Experienced some difficulty with trying to apply for SNAP and HEAP with the same application when HEAP starts. Recommends people to fill out form twice. Once for HEAP and once for SNAP to make sure information gets processed. Encourages registrants to write down Application Number.

JoAnn Zales spoke about HEAP opening on Monday 11/17 for primary benefits.
Emergency Benefits do not start until January. May apply on-line. JoAnn handed out a Home Energy Assistance Program Desk Guide as a reference along with a Care and Share Energy fund hand out.

Susan Dornan spoke about Caring for Ourselves-Caregiver support group.
Reminded everyone about Long term Care Council Meeting Dec.9th from 10a-noon.

Kim Sopczyk talked about Gift Back evening being held at Crandall Library on December 5th and 6th. Attendees can purchase gifts in the form of donations to give to loved ones whom have everything. Example $16 donated can purchase 100 lbs of food. There is also a SOCK DRIVE being done by Cornell Cooperative Extension and 4H to benefit those people who are in need of socks and shoes this winter.

Kathy Wallace spoke about Community Health Insurance Nov 15th SAIL will be open to help individuals fill out open enrollment applications. November 11th and November 24th SAIL is offering an education/meeting with a lawyer to discuss decision making. November 11th is based on Medicaid and November 24th is Advanced Directives, wills and such.

Nick George spoke about Basket Raffle going on November 15th at the Senior Center. November 25th is Adopt a Senior @ Queensbury Center.

Sue Gililngham reminded everyone about the raffle tickets for the Vacation to St. Maarten in April. Drawing is December 10th.

Interagency Council Meeting Minutes 10/9/14

Interagency Council Meeting Minutes: 10/9/14
Our next meeting will be held at the Warren County Human Services Building. Rebecca Smith from Glens Falls Hospital Smoking Cessation Program will be presenting about this FREE program. Kim Cook from Code Blue will be attending to talk with us about what that program is currently in need of. And there will be a HEAP update at the upcoming meeting as well.
Ally opened the meeting by reading our mission statement.
Community Connections Day – We are looking at making changes, most notably in the time of year as well as venue. We discussed the Washington County Fair, a grocery store, Walmart parking lot, Warrensburg Garage Sale, and the Balloon Festival. It was also noted that Worlco will no longer be a sponsor of this event. In order to address these aspects more fully, the Community Connections Day Committee will begin meeting immediately after the November meeting. Anyone interested in participating should plan on staying.
The Have a Heart application is on our blog. If you are interested in applying for your non-profit agency, please get the completed application in by next month’s meeting.
Attendee introductions and Agency Announcements:
Council for Prevention: The next meeting of the Suicide Prevention Coalition is on 10/28 at 10am, at the CFP.
SAIL – has begun a new program called Community Health Advocates (CHA), to assist people with health insurance issues.  SAIL fundraiser – “Sail On” will be held on 10/24, $50/ticket. Other news – Another new program, the “Stay At Home” project, will employ volunteers to do small at-home projects for seniors in need in Warren and Washington Counties (though they are hoping to grow from here). An informational session will be held at SAIL on 10/15 at 930am for those who wish to hear more about this new program.
House of Grace –Volunteers needed. They are also having a fundraiser on 10/18 at the Civic Center – Hockey game between the Flames and Phantoms. $20/ticket.
Catholic Charities of Warren County – offering free assistance for SNAP application.  Call Genny O’Brien at 424-7525 if interested in learning more.
National Grid – HEAP begins on 11/17
Hospice – raffle tickets for all expenses-paid trip for 2 to St. Maartins. Tickets are $25. They will also be holding “Hope for the Holidays”, a workshop on coping with loss, at the Queensbury senior center on 11/13.
Nick mentioned an opportunity for Interagency Council to promote themselves as well as individual agencies through a possible radio show – he will get back to us with more information.
Another point was brought up – with so many events lined up by various organizations, we find that there are often programs being held on the same days. We will start to compare planning calendars in order to combat this.
Today’s Presenter – Amy Alphonso from Mediation Matters.
 Since 1979, Mediation Matters has grown to cover a 6 county area including Albany, Rensselaer, Schenectady, Saratoga, Warren and Washington counties.
Mediation is a voluntary, private and confidential process facilitated by a trained and impartial mediator. The mediator hosts a conversation with two or more people in order to resolve a dispute and develop a mutually acceptable solution. Mediations Matters is funded by the NYS Unified Court System and has been around since 1979. Along with mediation, this center offers conflict resolution education, teambuilding and leadership development.
Situations best suited for mediation are those in which:
-          The issue involves strong emotions
-          The parties know each other
-          The parties want to maintain the relationship
-          One party is uncomfortable confronting the other side
-          The parties have reached an obstacle in trying to resolve the issue
-          The parties want to avoid costly legal litigation
-          The issue does not involve class action/punitive damage
However, mediation is NOT a place to seek legal advice or psychotherapy/counseling. It is not a court proceeding, and perhaps most importantly, it is not a place for situations with safety concerns, power imbalances or where there are allegations of ongoing child abuse/neglect and/or domestic violence concerns. The staff will refer a situation to another resource/avenue of intervention if it is deemed inappropriate for mediation.
All mediators are volunteers and go through a training certified by New York State (consists of 30 hours) and have observed and participated in mediations as part of their training.

Interagency Council Meeting Minutes 9/11/14

Chair Allison Reynolds started by welcoming all attendees to the first Interagency Meeting of the season. 
First order of business was deciding where to have October's meeting, as the room at Warren County's Human Services building is not available. After short discussion, it was decided that our next meeting will be held at the Queensbury Senior Center.
Information was passed out to attendees about how to access meeting minutes on the blog:

Ally read our mission statement and mentioned that we are welcoming of new members, both from non-profit and for-profit groups.

A tentative schedule has been set for the upcoming year's meeting schedule. Not quite full yet, once it is, it will be posted on this blog!

Membership dues for this year are now due. The amount is $10 for the year. Treasurer Sue Gillingham sent around dues forms, and she will post them on the blog for all to access.
Have A Heart Scholarship – We are looking for volunteers for that committee. This is a small, annual scholarship that is awarded to a deserving non-profit to fund a project within that agency, for which they would not otherwise have the funds. The scholarship application will be posted on this blog for access. The due date for the app is the Interagency November meeting, 9/13/14. Please see Sue Gillingham if you are interested in participating on this committee.
Treasurer’s Report – Currently we have $2389.28 in our account.  This money is used for Community Connections Day and the Have A Heart Scholarship.

Members introduced themselves and made Agency Announcements:
High Peaks Hospice: raffling of trip to St. Maartin’s and also having a golf tournament fundraiser The 6th Annual Claire Sweet Memorial Golf Tournament in Lake George. High Peaks is also continuing their volunteer recruitment, and offering free trainings at the Glens Falls Cancer Center Library. SAIL: The latest fundraising event for SAIL is being held on October 24th at The Hiland in Queensbury. Tickets are $50 each. They are also raffling off an IPad Mini!
SAIL also has a new logo which they are starting to use!

AIDS Council – Changing its name to The Alliance for Positive Health.
NY Connects – in conjunction with High Peaks Hospice, will be starting a informal caregiver support group the first week of October.
Family Services Association of Glens Falls – Hold their Meet the Medium night again, with Georgia O’Connor on November 8th 2014 at the Wood Theater. Tickets are $30 and are already selling, with a good chance they will sell out!
National Grid: Will have more info for us at the next meeting regarding HEAP. Joanne mentioned VET HELP (Veterans Housing, Employment and Life Skills Programs) as being a wonderful organization for veterans needing financial assistance and services.
Cornell Cooperative Extension – Has a new email – warren.cce@cornell.edu. Also has a new Ag Exchange, which is like a Craigslist for local farmers. For more info, go to http://agexchange.cce.cornell.edu/.
Big Brothers Big Sisters of the Southern Adirondacks – Having their annual golf tournament on September 13th in Stony Creek. They are also, holding a Country Western Night featuring the Vintage Country Band on October 3rd from 7-10, at the St. George Orthodox Church in South Glens Falls. Tickets are $15.
Ally mentioned that there is a grant writing workshop being held in Albany on 10/9 from 9a – 4p at the Wildwood School. Cost is $125 per person. For more information, please call 581-8841 or visit www.nonprofitworks.com.
Prevention Council – September is Suicide Prevention and Recovery Month. They will be holding a FREE showing of the documentary The Anonymous People at Crandall Library on 9/17/14 from 6p-8p.
Ally has also received a grant to begin a suicide prevention coalition for Warren and Washington Counties. This region is in the top 20 for highest rates in NYS for suicide. The first meeting of this coalition will be held on September 30th from 10a-12p at the Prevention Council.
GT Mobility LLC, MV-1 of Upstate New York – Matt Brown attended the meeting for the first time and presented some information on his business in Greenwich, which specializes in wheelchair accessible vehicles, lifts, ramps, hand controls, transfer seats, wheelchair carriers and any equipment needed to maintain one’s independence. Some discussion was had about nominating Code Blue for a Have a Heart Scholarship, but we are unsure of what they may need. Ally will call Kim Cook about possibly attending our next meeting to give us more information and let us know what they are looking for in terms of supplies and funds.
Community Connections Day – As we look toward planning another event, Ally would like us to think about the following questions, and be prepared for some discussion at our next meeting:
Do we want to keep Community Connections Day in the spring, or do we want to look at doing it in the fall?
Do we want to continue to partner with the Rotary or look for another event/organization to pair with?
 Finally, Crandall Park is not easily accessible for seniors or folks with mobility issues. Should we look for another place to hold it?

Hi everyone,
I forgot to add to the post I sent out yesterday. For agencies that are interested in having a table at the Community Forum Event on July 23rd  at the Hudson Falls High school. 
We will have the building open at 5:30pm to set up.
The event is from 6pm-8:30pm
If you are only able to stay for the beginning to provide resources at your table while people are coming in that is fine.

Congratulations to our Have A Heart Scholarship Winners!


Congratulations to High Peaks Hospice & Palliative Care and to Council for Prevention, who were both awarded Adirondack Interagency Council's Have A Heart Scholarship.
 
HPHPC was awarded $100 to purchase 100 "Five Questions" forms which assist people in planning and preparing for very important end of life decisions, for themselves and for their loved ones.
 
Council for Prevention was awarded $200 for their Youth Training Program which trains students to be peer leaders in the area of alcohol and substance abuse prevention within their schools, among their peers.

Interagency Minutes, June 12, 2014

  1. Interagency Announcements:
    1. This was the final meeting of the season, so we will re-commence meetings on the 2nd Thursday of every month starting with Thursday, September 11, 2014 at 8:30am, still at the Warren County Municipal Center off of Route 9.
    2. Membership is available to both for-profit and non-profit organizations within the human services community.
    3. Community Connections Day 2014 raised $750, $175 of which came from the Raffle Basket. Our current balance is $2389 which will go toward next year's Community Connections Day, as well as towards our Have A Heart Scholarship. About 200 people attended due to better tent location and more foot traffic as a result.
    4. Interagency awarded their Have A Heart Scholarship to High Peaks Hospice & Palliative Care ($100 for Five Questions Form) and $200 to Council for Prevention for their Annual Youth Training Program around preventing alcohol & substance abuse among their peers by becoming peer leaders. (pictures to be posted immediately following Minutes).
  2. Agency Announcements
    1. Allison Reynolds of Council for Prevention announced a program will be held at their office (10 LaCross Street, HF) on June 23rd at 11am regarding opiate/heroin addiction, awareness, resources and education around those issues.     2. Nick George of Queensbury Seniors announced their annual Community Health Fair, sponsored by Senator Betty Little,  on September 13th from 10a-2p at Queensbury Senior Center, 742 Bay Road in Qbury. Betty Little & Dan Stec will be in attendance, as will various "information/support vendors". Screenings for hearing, blood pressure & glaucoma will also be available.     4. Phyllis Morreale of HHHN's Teen Awareness Group announced a new one week health club (FREE) for youth ages 12-15 called "Getting Ready" (flyer will be posted immediately following these Minutes).     5. Renee Stephenson of AIDS Council of Northeastern NY announced that after 30 years of serving those with chronic diseases, they are undergoing the process of a name change effective end of June. Because of their current name, the public does not know that their organization can assist ANYONE living with ANY CHRONIC DISEASE, not just AIDS. The name change will encourage more people to utilize their resources, though the process of the name change has been a painful one.
    6. There are several trainings currently available around narcotic/opiate/heroin addiction, including the use of NarCon kits. For information about these trainings, contact Allison Reynolds at Council for Prevention: allisonreynolds@councilforprevention.org
    7. Sharon King with Catholic Charities Domestic Violence Project spoke about a 2008 law that allows youth under the age of 15 to get an Order of Protection through Family Court with no permission needed from their guardian(s). The purpose is to address the prevalence of dating violence among youth. More info contact sharonking1@yahoo.com
    8. Jackie Dunn announced the Soroptomist Garage Sale on June 20 & 21 on Chestnut Ridge in Queensbury. Half benefits Catholic Charities Domestic Violence Project, and half benefits Soroptomist Scholarship Program. Good stuff for good causes! Info: majadunn210@gmail.com
    9. Susan Dornan of NY Connects announced that all are invited to the Long Term Care Council meetings, held quarterly. Purpose is to address gaps and barriers to services by seniors. Info: 1-866-805-3931 or 518-746-2420.

    10. Gail Clark with Rehabilitation Initiatives announced a new program by US Social Security called "SSA Ticket to Work" to help those on disability go back to work without losing their benefits.     11. Jackie Dunn with Neighbors NY announced that there are still open positions available, and she HIGHLY recommends volunteering at House of Grace.
    12. Audrey Shaw with Big Brothers Big Sisters of the Southern Adks announced that their 18th Annual Golf Tournament sponsored by Girard Landscaping raised $25,700, and that Tackett Chiropractic's "Run for the Ages" would be held on Sat June 14th benefitting both BBBS and GF Senior Center.

    1. Warren Washington County Council for Prevention uses evidence-based programs in schools to prevent substance abuse and foster healthy communities, schools, families and individuals.
    2. MADD Power of Parents Program features a 20 minute training for parents on how to talk with their kids about drugs & alcohol, which significantly reduces occurrence of substance abuse later in their life.
    3. Peer-to-Peer "Natural Helpers" groups in schools program seeks to identify the informal "helping network" of youth within their school and provide training/support to those students helpers (& teachers) to strengthen the efficacy of that network within the school.
    4. Gambling-surprisingly to some, youth are highly susceptible to gambling addiction due to the fact that their pre-frontal cortex is not fully developed until the age of 25, which means their are more apt to take risks and make poor judgements around substances and risk/reward decisions.
      1. With the proliferation of Facebook/computer games based on earning "jewels" to build bigger & better "houses", etc., and to earn points toward actual prizes, their developing synapses are being "programmed" for high reward, very quickly with little effort and "high novelty" experiences.
      2. Youth & Seniors are more susceptible to gambling addiction.
      3. The introduction of casinos into new communities results in increased domestic violence.

Kim Sopczyk of Family Service Association, serving Warren/Washington/Northern Saratoga Counties, was holding an open house for their new location at 150 Warren Street in Glens Falls on Friday, April 11th from 9a-11a. In 2013, they served 8000 members of our community! Infant needs, food pantry, holiday program, special needs program, emergency dental care, Feet First program, vision & hearing screening, & more.     12. Traci Ross, Executive Director of Big Brothers Big Sisters of the Southern Adks, announced a new pilot program at South High geared towards reducing bullying and lowering suicide rates. Juniors will mentor Freshmen for 2 years, being their Buddy in school, and once the Freshman become a Junior, they will mentor a Freshman. If successful at South High, Q'bury & LG High will adopt the program next.
      1. She also announced that their Bowl For Kids' Sake this year raised $56,000 - which make them one of the Top Five earners!

    1. Mission: Reduce trauma to child victims of physical & sexual abuse and their non-offending family members by working with a multi-disciplinary team of experts, who specialize in child abuse, from both Warren and Washington Counties.
    2. They complete systematic forensic interview right in their welcoming facility on Dix Avenue, which are admissible in court.
    3. Their systematic approach has changed the child abuse system by reducing the number of professionals that child victims have to talk to from FIFTEEN down to THREE, streamlining the prosecution process.
    4. The Adk Chamber of Commerce's "Leadership Adirondack" Class of 2014 held fundraisers to allow C.A.R.E. to purchase state-of-the-art recording equipment which is easier to use, and which provide high quality sound recording--much more effective for use in court.
    5. Senator Betty Little's office awarded them $100,000 as well.
    6. WAIT House/C.A.R.E./YMCA are partnering to provide a Healthy Parenting & Mentoring Program for teen parents, who MUST commit to this program for one year.

Next Interagency Meeting

Good Morning!

I just wanted to let everyone know that our speaker for the next Interagency meeting is Kassia May from Warren Washington CARE Center. Our nest meeting is April 10th  At 8:30 in the Warren County Human Services Building first floor conference room with the kitchen.

Also we will have a Community Connections Day meeting directly after the Interagency meeting is over. This will be our last meeting before the event so it will be the time to get everything wrapped up.
If anyone is interested in donating a basket for the raffle please let Sue Gillingham or me know.
I will also have the Community Connections Day Posters to give out for promotion of the event.

    3. Kim Sopczyk of Family Services announced that they will be moving into their own building on February 1st, 2014 at 150 Warren Street in Glens Falls. They have been in the Glens Falls Association for the Blind building on Ridge Street, 2nd Floor, for quite some time.
    4. Phil Jessen of Glens Falls Association for the Blind announced that they now have that space available for lease to a non-profit as a result of Family Services departure, terms negotiable, heat included:
      1. 1600' square feet, 2nd Floor, 144 Ridge Street, Glens Falls
      2. Off- and On-street parking
      3. Separate front & rear entries
      4. Large conference room, 3 offices, full kitchen/dining room, restroom

    2. Liz Mahon-Laidlaw, SAIL Center-Outreach Coordinator for the grant that SAIL was awarded to assist people in signing up for the Affordable Health Care Act's insurance marketplace.
      1. Liz announced that there are positions available associated with this Grant for the Navigator Program. Contact Anna Livingston at 792-3537 for more details.

    1. Audrey Jean Shaw, Director of Fundraising & Community Outreach with House of Grace of the Adirondacks (HoG), a house which provides 24/7 supervision for those traveling life's last journey who are also on hospice-"Though we cannot add days to their life, we can add life to their days."
      1. HoG was the dream of a Hospice Nurse named Mary Ann Woodward who saw the need for a place in our community for people to live who are on Hospice but who do not have home or family support to travel life's last journey outside of a hospital or nursing facility. (Hospice does not provide 24/7 care--individuals must pay for in-home 24/7 end-of-life care). HoG does not provide medical care, as High Peaks Hospice & Palliative Care provides that.
      2. Finch Pruyn bought the house at 33 Henry Street, and Tom Albrecht of Hilltop Construction gutted & re-stored the home. HoG doors were open in 2005.
      3. HoG receives no local, state or federal funding and is completely dependent on community donations. On April 1st, 2013, a new sliding scale donation "ask" was added to the intake process. If families can afford to make a donation, they do. If they cannot, they are not required to do so. Entrance to HoG is based on need.
      4. Since 2005, over 50 people and their families were served by HoG, 8 of whom have lived at HoG since April 1, 2013.

  1. Interagency Updates
    1. Ginger Lacross of The Cedars attended as a guest. Welcome Ginger!
    2. CONGRATULATIONS! The S/W/W Interagency Council awarded the 2nd Annual Have a Heart Scholarship, totaling $350, to two organizations: High Peaks Hospice & Palliative Care and The Council for Prevention.
      1. High Peaks was awarded $100 to purchase 100 copies of the "5 Wishes" pamphlet to distribute/fill out with members of the community so that they have a Health Care Proxy and Living Will in place. The pamphlet also contains information about why this is beneficial and necessary.
      2. The Council for Prevention was awarded $250 for their annual Students Against Destructive Decisions conference so that they don't have to limit the number of SADD student members from 11 area schools who can attend this day of learning & training. Participants become Student Leaders in their schools/communities, bringing with them awareness-raising around the issues of leading healthy lifestyles and making good decisions about alcohol use, drugs and texting while driving.
  2. Speaker: Joan Prouty (dprouty@capital.net), Cornell Cooperative Extension, VITA Program
    1. VITA stands for Volunteer Income Tax Assistance & provides FREE tax return assistance for eligible residents of Warren & Washington County (and in some cases VT).
    2. This program comes from the "kinder, gentler side" of the IRS and started 11 years ago.
    3. Its aim is to help eligible taxpayers (household income $51,000 or less) file their Income Taxes for FREE, to claim their Earned Income Tax Credit if applicable, and to keep all of their refund instead of just part of it (HR Block, et al, charges fees for filing and instant-refund programs).
    4. The Earned Income Credit MUST be applied for on the Income Tax Form, & VITA helps eligible taxpayers do so, for NO FEE.
    5. Appointments are REQUIRED.

    6. There are 12 locations in Warren County and at least 3 in Washington County.
