Judith asked the group if they had any input on how the Community Connections day went. If anyone wants to give some suggestions on how we can improve the day please post comments on the blog or email Judith McKinnon at JMCK@sail-center.org.   
Monthly Speakers
Southern Adirondack Independent Living Center:  The independent living center provides services to those with disabilities so that they may be live more independently in their homes and communities. Some of the services that were presented on are advocacy, information and referral, architectural barrier consultation, peer counseling, community education, free loan of assistive equipment such as wheelchairs or shower seats, early intervention equipment program, adaptive computer equipment and classes and the TBI/NHTD Regional Resource Center which administers the TBI/NHTD Medicaid Waiver Programs. All of our services are free.
SAIL also has a fully adaptive kitchen and meeting rooms for any group who needs an accessible place to have gatherings or meetings. Christine Stegel  presented from IPRO:
IPRO’s goal is to improve healthcare for the common good. IPRO is a not-for-profit company under contract with the Centers for Medicare and Medicaid Services. They work to improve the quality of healthcare received by Medicare beneficiaries across NY.  Christine discussed Care Transitions which are a set of actions to ensure the coordination of healthcare programs as they transfer between different healthcare locations or levels of care in the same location.
